Team Member Performance Review

I helped design a Team Member Performance Review Form to support the company’s end-of-year evaluation process. The organization had been experiencing challenges with employees completing reviews on time and feeling unsure about how to prepare for their performance discussions. To address this, I created a structured, easy-to-use form that could be distributed one month before review meetings. The goal was to help employees reflect on their progress, identify key accomplishments, and come prepared with meaningful talking points for their conversations with supervisors.
